Extracoronal attachments for partial dentures

Various attachments can be used for fabricating patient-removable dentures. Custom-made or prefabricated components are used. During clinical recall appointments it was documented that long-term clinical success of removable dentures is primarily determined by biological factors (1, 2). As a consequence, discussions about the advantages and disadvantages of various types of denture, influenced by the mainly mechanical way of thinking, were forced into the background. Failed treatment due to periodontal complications and/or caries can only be avoided by effective pretreatment and postoperative patient recalls. The patient should attend a recall appointment at least once per year and be motivated regularly to assume personal responsibility and comply – these are essential for proper aftercare treatment. The application of fluoride or chlorhexidine should not be underestimated for backing up oral hygiene, above all due to the continually rising age of the population (3). Therefore, during the reconstruction phase, the technical aspects are limited to evaluating the biological and mechanical feasibility of the abutments as well as integrating the denture into the stomatognathic system so as to function properly and without causing trauma. Aftercare treatment for dentures impresses on both the dentist and patient that regular servicing of all removable dentures is of paramount importance. It is extremely important that incorrectly functioning dentures be repaired as quickly as possible.
